- Medication refill form — the patient's name, date of birth, phone number, email address, medication and dose, preferred pharmacy, and any notes you add.
- Newsletter signup — your name and email address, so we can send you practice news and mental-health resources.
- Analytics — if you consent to analytics cookies, general usage data such as pages viewed, approximate region, and device or browser type.
How we use your information
We use the information you submit to schedule and manage appointments, answer your questions, and process medication refill requests for established patients. With your consent, we use analytics data to understand how the site is used so we can improve it.
If you subscribe to our mailing list, we use your name and email address only to send you practice news and mental-health resources. You can unsubscribe at any time by replying to any email we send, or by contacting us using the details below. Subscribing is never required to receive care, and we do not add patients to the list automatically.
We do not sell your information, we do not use it for third-party advertising, and we do not build marketing profiles from it.
How your information reaches us
Form submissions are delivered to our clinical team by email through Google Workspace (Gmail), our email provider. Information you send through a website form is not a substitute for your medical record, and this website is not a patient portal.
Please do not include detailed clinical or sensitive health information in the free-text fields — send only what we need to reach you and route your request.
How we protect your information
We implement appropriate technical and organizational measures to protect your information against unauthorized access, disclosure, alteration, or misuse. The site is served over encrypted connections (HTTPS), and access to submitted requests is limited to staff who need it to do their jobs.
No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.
How long we keep it
We keep website inquiries and refill requests only as long as needed to handle the request and meet our record-keeping obligations. Mailing-list details are kept until you unsubscribe. Information that becomes part of your medical record is governed by our HIPAA Notice of Privacy Practices rather than by this policy.
Your rights
